The Passport Services program is no longer accepting appointments.
Please have all required documentation with you at your appointment; your application cannot be processed without these:
- Form DS-11 (completed, but not signed)
- Proof of U.S. Citizenship (original birth certificate + photocopy)
- Proof of Identity (original driver’s license + photocopy)
- Two separate payments* (check or money order, see Fee Chart)
- One recent 2″x2″ color photo (photo services nearby at CVS Pharmacy)
*One check/money order made to out the U.S. Department of State
and One check/money order made out to Concord University

Payment Instructions: The application fee is paid directly to the Department of State. The Execution/Acceptance fee is paid directly to the acceptance facility. Two separate payments are required. (Effective 1/22/2023)

Q: What is the difference between the passport book and the passport card?
A: The card can be used only for land/sea travel to Canada, Mexico, Bermuda, and the Caribbean. The book can be used for travel to any destination, by any method of travel.
Q: Are notarized copies of citizenship evidence accepted?
A: No, only original or certified copies are acceptable. Citizenship evidence will be returned to you via First Class mail. For more info on required documentation, visit the official Department of State website.
